You can also provide a phone number, company website, and notes. Please do not submit patient information, health records, payment details, or other sensitive personal information.
Website and campaign information
The website retains the first landing page, available campaign parameters, and referring page for the browser session. It uses lightweight platform analytics to understand page visits and interactions such as review-button clicks, form starts, and form submissions. Contact fields and free-text responses are not intentionally included in custom analytics events.
